Title: Enrollment Case Manager
Location: Lackawanna County, PA (Hybrid/On-The-Road)
Department: Pennsylvania Independent Enrollment Broker (PA IEB)
Work Hours: 8:30am - 5:00pm
Training: Four weeks of training
Pay: $20.50/hr or $21.50/hr Bilingual (Spanish)
Reimbursement: All travel expenses reimbursed (gas, parking, tolls, etc.)

About The Role
The Enrollment Case Manager (ECM) plays a key role in supporting Pennsylvania’s Home and Community-Based Services (HCBS) through the Independent Enrollment Broker (IEB) program provided by Maximus. ECMs assist seniors and individuals with disabilities by conducting in- home intake visits, guiding applicants through the enrollment process, verifying eligibility, and handling necessary documentation.
All technological needs will be provided by the company. After successfully completing a contingency period of 120 days, we have an excellent benefits package, full insurance, and benefits on the first of the month following the end of the probation period. We also have a bonus structure for all employees.

Your Impact:
As an Enrollment Case Manager, you’ll help seniors and individuals with disabilities access the care they need. In this role, you will:
  • Conduct in-home visits to guide applicants through enrollment and eligibility
  • Verify documentation and maintain accurate case records
  • Provide unbiased information on program options and applicant rights
  • Build relationships with families, providers, and community organizations
  • Identify and address barriers so applicants receive timely support
  • Conduct in-home intake visits, assessments, and application support for waiver eligibility.
  • Travel to applicants’ residences, including nursing facilities and shelters, ensuring documentation is complete and accurate.
  • Follow up as needed to assist applicants in meeting eligibility requirements.
  • Perform outreach and administrative tasks when not in the field.
  • Maintain professionalism and ethical standards, acting as a mandated reporter when necessary.
  • Use Microsoft Office Suite, electronic documentation, and mobile technology for reporting and data entry.
  • Attend required meetings, training, and team discussions.
These responsibilities are not meant to be all-inclusive; additional tasks may be assigned.

Minimum Qualifications:
  • Bachelor’s degree with at least 12 credits in sociology, psychology, social welfare, gerontology, or related field and one year of experience in social work or case assessment; OR
  • Two years of casework experience, including at least one year assessing individuals for institutional or community-based services
  • Valid driver’s license and reliable transportation for frequent travel within assigned county
  • Strong organizational and communication skills with the ability to work independently
  • Proficiency with Microsoft Office and ability to use electronic documentation systems
  • Ability to maintain confidentiality and adhere to ethical and professional standards
  • Must pass required background checks and have no affiliations with OLTL providers


Standout Qualifications:
  • Bilingual skills (Spanish/English or other languages) to better serve diverse communities
  • Prior experience in healthcare, Medicaid/Medicare, long-term care, or disability services
  • Knowledge of Home and Community-Based Services (HCBS) or waiver programs
  • Familiarity with HIPAA regulations and handling confidential or sensitive information
  • Experience working in field-based or hybrid roles requiring travel and self-management
  • Ability to build trust quickly and maintain professional boundaries while showing empathy
  • Experience collaborating with community organizations, advocacy groups, or service providers
  • Prior use of case management software, CRM tools, or electronic health records


Physical Requirements

  • Frequent Driving: Ability to drive to and from applicants’ residences and various field locations within and occasionally beyond the assigned county. This requires sitting for extended periods, maintaining alertness, and operating a vehicle safely.
  • Mobility and Stamina: Ability to enter and exit a vehicle multiple times a day, walk short distances, and navigate a variety of environments (including private homes, nursing facilities, and shelters).
  • Sitting/Standing: Ability to sit for prolonged periods while driving or completing documentation, and to stand during in-home visits and assessments.
  • Handling and Dexterity: Frequent use of hands and fingers for operating a computer, mobile devices, and handling paperwork or documentation.
  • Lifting: Ability to lift, carry, push, or pull objects such as a laptop, portable printer, or case files, typically weighing up to 25 pounds.
  • Vision and Hearing: Sufficient vision for reading documents, screens, and driving; adequate hearing for communication in person and by phone.
  • Communication: Clear verbal communication and adequate hearing for effective interaction with applicants, families, and team members.
  • Work Environment Adaptability: Ability to work in a variety of settings, including private residences, facilities, and sometimes in less-than-ideal conditions (such as limited space, varying cleanliness, or environmental distractions).
  • Weather and Environmental Exposure: Willingness and ability to travel and perform duties in various weather conditions and environments encountered during fieldwork.
